Mobile Community CareLink (CCL)

Description

Mobile CCL is powered by First Call's shared electronic medical record (EMR), Community Care Link (CCL), which is specifically designed for behavioral health agencies. Mobile CCL serves as a web-based client portal into the CCL database and is expanding and enhancing the community's ability to coordinate recovery, mental health and family services for uninsured individuals with co-occurring mental health, substance use disorders and/or primary healthcare issues. Mobile CCL is a recovery and support tool to use on one's path to healing. Within the Mobile CCL portal, clients can take charge of their treatment and recovery process by updating contact information, commenting on and closing out treatment plan tasks, completing assignments and sending secure messages to their clinician. The web-based behavioral health record is accessible by multiple agencies serving the same client. Each client's shared behavioral health record is a complete record of the client's coordinated care and includes the following: intake; screening (CAGE-AID, Modified Mini Screen); triage; assessment (ASI, SASSI, SOCRATES); referral and appointment setting; diagnostic, person-centered and culturally-specific recovery planning; and transition planning. With the help of Mobile CCL, clients and clinicians can work in partnership towards recovery.

MobileCCL.org is the public website that offers a variety of resources, including registration for a Mobile CCL portal account.

Goal / Mission

The goal of Mobile CCL is to foster a higher level of client engagement and motivation in the treatment/recovery process.

Impact

As a web-based client portal to care management, Mobile CCL is enabling the integration of behavioral health treatment with primary care.

Results / Accomplishments

CCL is positioning Kansas City's behavioral health community for integration with the primary health care community. The use of client portals is common among primary health physicians and insurers, but is innovative in behavioral health. In 2013, 309 clients engaged via the Mobile CCL web portal.
