Goal: Combat chronic diseases in Miami-Dade County though innovative screening and education initiatives based upon personalized hereditary and lifestyle factors
The Healthy Hub evaluation will utilize data collected through the screening process and Life Simple 7 surveys administered at the time of screening. The screening will include the following data:
Nutrition
Tobacco use/Smoking
Physical Activity
Body Mass Index (BMI)
Cholesterol
Blood Pressure
Blood Sugar
Waist Circumference
Body Fat Percentage
As a result of the assessments, individuals will receive recommendations from linkage to care specialists/registered nurses (RNs) to better health outcomes and will be linked to providers and medical homes through community referrals. Additionally, participants will be given "Green Prescriptions" as appropriate, which are "prescriptions" to engage in more physical activity while identifying community resources where residents can achieve this objective. Participants will be encouraged to return at 6 months and 12 months for follow-up screenings and to track progress.
