Ranking Effective Practice
Description The Neighborhood Health Clinic (NHC) is a medical home for uninsured, working adults employed at or below 150% of the Federal Poverty Level. Patients receive a nursing assessment, physician visit, in house laboratory services, and one month of prescription medications in exchange for a $20 contribution. Referrals for tests and exams are offered at no charge, and all physician and hospital services are donated at no cost to the patient. NHC also focuses on health promotion and chronic disease management, with educational programs in Diabetes management, Hepatitis, healthy lifestyles, colon cancer screening, breast health, ad social services. Monthly specialty clinics for medical conditions include cardiology, dental condtions, dermatology, endocrinology, otolaryngology, gynecology, hepatology, lipids, neurology, pain management, rheumatology, and urology. The program is funded through community and individual donations, the United Way, foundation grants, businesses, and religious and civic organizations.
Goal / Mission The goal of the Neighborhood Health Clinic is to provide medical care to low-income, uninsured, working adults.
Results / Accomplishments The Neighborhood Health Clinic has been recognized locally and nationally for the services they provide. The organization has expanded to include 250 volunteer physicians, 100 volunteer nurses, and 150 non-medical volunteers. During a typical week more than 200 patients are treated and over the 2007-2008 fiscal year 4,174 patients received over $950,000 in services and received 14,429 prescriptions worth $1.2 million. Since its inception, NHC has served nearly 34,000 patients at a value of $8.6 million.
