The MD-HAN Community Health Worker/Patient Navigator (MD-HAN CHW/PN) Work Group of the MD-HAN represents the collective voice of Miami-Dade County’s CHW/PNs and is focused on bringing awareness to their critical work. The MD-HAN CHW/PN’s primary goal is to develop a platform for the exchange of best practices and lessons learned. This information is assembled and disseminated throughout the Network in order to contribute to the enhancement of service delivery among CHW/PNs. The efforts of the MD-HAN CHW/PN efforts are aligned with the Florida Community Health Worker Coalition, the statewide partnership dedicated to the support and promotion of the CHW profession in Florida. The Work Group aims to accomplish the following objectives:
- Bring awareness to the work of CHWs/PNs in Miami-Dade County in order to support the formalization process
- Review national CHW netowrk models and assess its replication in Florida
- Standardize a CHW definition and scope of practice
- Make advancements in creating a statewide certification initiative
- Identify sustainable financing streams and reimbursement mechanisms
- Support the implementation of a training and certification process
Building upon the work on the 2014 Florida Community Health Worker (CHW) Census, the Health Council of South Florida in partnership with the Florida Department of Health, recently shortened and refined the census survey to get the best data possible. By completing this survey, you will enable us to enhance the training opportunities available to CHWs, and the capabilities of the Florida CHW Coalition.

Florida CHW Certification Process
The Certified Community Health Worker (CCHW) designation is an entry-level credential for front-line health workers who, by virtue of their trusted status in the community, serve as a liaison, link and intermediary between health services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Examples of job titles that are considered to be Community Health Worker positions include but are not limited to Promotores(as) de Salud; Community Health Educator; Health Communicator; Outreach Worker; or Health Advocate.
The CCHW credential is open for grandfathering from January 1, 2015 through June 30, 2016. The purpose of a grandfathering period is to provide current practitioners with the opportunity to earn certification without taking additional training or the written exam. Grandfathering period certification standards include a combination of specified work and/or volunteer experience, content specific training, and letters of recommendation. Applicants must provide formal, verifiable documentation to support each requirement.
There is a $50, non-refundable application fee.
There is a $25 hard copy application fee (optional — only required if an applicant DOES NOT apply on-line).
There is a $20 criminal background report fee (must be paid by any applicant who has had a Felony or First Degree Misdemeanor charge).
